The Assertive Community Treatment (ACT) Team works only with clients who have a severe and chronic Axis I diagnosis (schizophrenia and other psychotic disorders, major affective and bi-polar disorders). ACT clients typically are unable to engage in traditional mental health services, are non-compliant with medications, have numerous hospitalizations and demonstrate significant functional impairments in accessing safe, affordable housing, avoiding destructive lifestyle choices and have difficulty completing practical daily living tasks.
